Policies & Financial Arrangements

ADMISSION:

 

Oak Lane is a community oriented 60 bed skilled nursing and rehab facility giving the best possible care regardless of race, color, creed or religion. Before admission our Director of Nurses and our Social Service Supervisor will meet with you, your family, and physician to make sure we can provide the services you may need. There is a possibility that we would not have an immediate vacancy; you would, however, be placed on a waiting list. The facility reserves the right to transfer any resident from one room to another or from one bed to another when advisable. The resident or representative will be notified of such a move. With physician’s orders, residents may leave the facility with the family for a ride, outing, etc. Before leaving the premises, the resident must be signed out at the nurses station.

 

RESIDENT FUND ACCOUNT:

 

You, or your family, may place up to $50.00 in a personal account with our business office. This can be taken out upon request during business hours for any personal item you may need or want (i.e. stamps, special soap, shampoo, etc.).

 

CARE CONFERENCES:

 

You will be scheduled for a care conference quarterly: or more often if necessary. The contact person from the family will be notified and encouraged to attend the conference. Attending this conference will be supervisors from the nursing, dietary, administration, social services, physical therapy and activity departments. It is an excellent opportunity for your family to be involved with your care.

 

MEDICAL CARE:

 

Dr. Robert Pogue is our facility Medical Director. However, you may have your own physician if you so desire. If you choose to have your own physician, your family must provide transportation for monthly visits to the physician for the first three (3) months and thereafter once every sixty (60) days. The admission physical including lab work will be billed to Medicare and/or the resident or representative.

 

MEDICATIONS:

 

All medications are ordered from the pharmacy which Oak Lane has a contract. No medications can be brought from home, this is in accordance with Public Health regulations. The meds are administered only by the nurse on duty and are kept in locked carts or the Med Room at all times; these meds are ordered by your physician as necessary.

 

VISITING:

 

We encourage all family members to visit at any time. Please keep in mind that some of our residents need extra care getting ready for the day and some do like to retire earlier than others. We do request that any visitors be free of any communicable diseases. Also if you plan to take a resident out for a period of time , we would appreciate a call in advance so that we might have the resident prepared when you arrive.

 

SMOKING POLICY:

 

Oak Lane is a smoke-free facility. Smoking is prohibited by residents throughout the facility and grounds.

 

MEALS:

 

Everyone is encouraged to eat in the dining room. If you have a special diet, provisions will be made. Meal times are:

 

7:30 a.m.      -      Breakfast

11:30 a.m.     -       Lunch

5:15 p.m.      -      Dinner

 

If the resident chooses to eat in their room, the times above will vary depending on the room location. Your family is welcome to eat with you when their schedule permits. There will be a minimal charge for guest trays; arrangements should be made ahead of time. Please check with the nurse before your family brings food in for you. Food in the resident’s room which has been brought in by the family must be in containers with tight fitting lids. Periodic checks of the rooms will be made to make sure there are no food containers with food which should be discarded.

 

CLOTHING/HYGIENE:

 

You are encouraged to dress in your own clothing both day and night; shoes should be comfortable, sturdy and flat heeled, slippers should be the washable non-skid type. All clothing should be marked with your name; staff will be glad to do this for you. We will launder your clothes, at no extra cost, or your family may take your clothing home to be laundered. The admission nurse or Social Services Director will review what hygiene items you may bring with you from home. Some items are prohibited by state regulations.

 

BATHS/SHOWERS:

 

You have the choice of a bath, shower or whirlpool once a week, or as frequently as necessary. The staff will assist you, wash your hair and trim your nails.

 

BEAUTY SHOP/BARBER SHOP:

 

A beautician is available each week; the barber is available monthly. There is a separate charge for these services. The Activity Director will schedule appointments for you if you wish.

 

TELEPHONES:

 

Long distance calls may be made from the facility phone by calling collect. You may have private telephone line in your room at your own expense. Arrangements should be made by your representative directly with the telephone company. Contact Social Service Supervisor for additional information.

 

MONTHLY CHARGES:

 

Charges will be submitted to your family on the first of each month; payment is requested by the 10th of that month. In the case of Public Aid residents, you will not be billed, however lack of payment is determined an Abusive or Neglectful situation. In this case we must by law notify the Department of Public Aid. We regret the need to raise room rates, however, increased cost does require room rate increases on occasion. When this is necessary we will notify you a month in advance.

 

BED HOLD POLICY:

 

Private pay residents may hold a bed by paying 75 % of the room rate. Public aid residents may hold a bed for ten (10) days at Public Aid’s expense, after that time period the family must pick up the expense (75 % of the room rate) or we may release the bed to a new resident.

 

TRANSPORTATION:

 

Oak Lane Nursing and Rehab has two vehicles that we use for outings and to take residents to doctors appointments and other appointments when family members can not. Although there is no charge we would appreciate a donation that covers our expenses (staff and use of vehicles).

 

TELEVISIONS:

 

We have outlets in each room for television, however, we are not connected to a cable system, so channels are of a limited and local flavor. Please check with housekeeping because size of the set is important. We also recommend a set that has the capabilities of remote control. Top opening is 26 ˝” wide by 21” high. Lower opening is 26 ˝” by 14” high. Televisions may not be played loudly or past 9:00 p.m. unless it is agreed upon by both residents.

 

PERSONAL ITEMS OF VALUE:

 

As you enter the facility we inventory all valuable items such as cash, credit cards, jewelry, etc., so that we can protect you against possible loss of those items. This inventory will be done by the family member who admits you, our Supervisor of Housekeeping, and our Social Service Supervisor, we will of course inventory all other items at this time such as clothing, furniture, etc. We would suggest that you keep only a small amount of cash on you or in your room. As time passes and you bring in additional items of clothing and/or personal items, we would like for you to notify Housekeeping or the charge nurse so these items may also be labeled and inventoried. Approximately once a year we will attempt to sort thru items of clothing that may no longer fit or that need to be discarded. If any item is discarded you and your family will be notified. When rings or other jewelry become loose because of weight loss, etc., please advise the charge nurse that you are sending it home with your family so we may update our inventory.

 

PHYSICAL / OCCUPATIONAL / SPEECH THERAPY:

 

Our mission at Oak Lane Nursing & Rehab is to guide our residents with physically disabling conditions along an individualized treatment program so they may reach the highest level of physical, social and emotional well-being. To assist us in doing this we contract outside our facility for the best rehabilitation program that we can bring into our facility.

By combining the efforts of the resident, family, physician and rehabilitation professional we can design a multi-disciplinary team approach that will have optimal results.
